As announced in its May 2025 Network News, UnitedHealthcare has a new mailing address for contract notices. As required by the UnitedHealthcare Participation Agreement, all written notices must be submitted via certified or overnight mail and must now be sent to:

This address is ONLY for the submission of formal contract notices. Please do not use it for claims related issues or disputes. For claims, continue to use the address listed on the back of the patient’s ID card.
