The California Medical Association (CMA) is collaborating with Aledade Inc. to help independent physicians succeed in value-based care, ensuring practices and health centers are able to thrive while delivering the best possible patient care.
As the top-rated, physician-led Accountable Care Organization (ACO) in the country, Aledade’s proven process helps practices and community health centers quickly improve financial performance and get more from the data and technologies they have. This year, Aledade projects it will help practices earn more than $83 million in shared savings for their performance in 2020. Other highlights for these practices include:
- An average first year fee-for-service revenue improvement of 20%
- Avoided over 10,000 hospitalizations in 2019 via improved workflows and care management for hypertension, diabetes and cancer
- 100% of Aledade ACO members earned shared savings in their first year of entering Medicare’s new Pathways for Success program
For independent primary care practices and community health centers, the upside of adopting value-based care can be significant -- increased revenue, more efficient workflows, more accessible, impactful patient data. While data shows physician-led ACOs outperform other models, the process of transitioning is often more challenging for practices with limited human, technical and financial resources.
Primary care practices and community health centers considering joining an ACO can estimate their potential shared savings revenue with the Aledade Advantage calculator.
The deadline to join an Aledade ACO for the 2022 performance year has been extended to August 3, 2021. Physicians are encouraged to contact Adelade no later than July 31 to ensure your application can be processed in time.
