EDD updates paid family leave claim form

The Employment Development Department (EDD) recently made changes to the Paid Family Leave (PFL) benefits claim form (DE 2501F). Effective May 1, 2015, EDD will no longer accept the old claim forms for PFL benefits. The new claim forms are available to order on the EDD Forms and Publications page or you may call (877) 238-4373 to order more forms.

PFL is a component of the State Disability Insurance (SDI) program and administered by EDD. For California workers covered by SDI, PFL provides up to six weeks of benefits for individuals who take time off work to care for a seriously ill child, parent, parent-in-law, grandparent, grandchild, sibling, spouse or registered domestic partner, or to bond with a new child. EDD encourages you to submit claims and forms through our electronic claim filing system, SDI Online. The system allows physicians/practitioners, claimants, employers and voluntary plan administrators to submit claims and other supporting documents online.
